Mike,

I'm surprised major balls-ups didn't appear sooner, what with 1, maybe 2, year apprenticeships, 6 week training courses, plumbers in charge of engineers, and the like. A friend who left the MOD shipbuilding environment 10-12 years ago told me of asking a training course trained numpty why, in the pipe arrangement then visible on his screen, he had drawn the pipes snaking all over the place.  "That's what it shows on the diagram," was the answer.  He hadn't actually drawn the pipes, just brought them into being via software - my friend doubted if he'd ever seen a pipe, let alone a ship.
