This sort of thing can be done through (a) ignorance, or (b) political 
provocation. As to (b),one way to treat it would be to refuse to stamp 
erroneous passports, as is done for other errors. But mostly it's ignorance. 
We get used (really, we don't) to maps of the UK on film or TV with England 
printed (sideways) all the way from Kent to Rossshire.
