Colin,

You highlighted the main bits.

Fabricated bolts?  No wonder they were sub-standard.  Were they welded together with too thin welds?

What kind of engine were they holding down?  HD bolts holding down the frames?  Pretty long bolts if they were diesels.

There's only one way to install a fitted bolt - apply some approved "grease" and hammer it in.  How can it be improperly installed?

And finally - shear forces during a shock event.  Ramming the land?  The bolts on a direct drive diesel are subject to continuous shear forces from the thrust.  There may or may not be collision chocks to take abnormal forces.  Was it a metallurgical problem?

We once had a problem with some over-stressed cyl head studs on a Kincaid/B&W diesel.  All seemed kosher until someone thought to check the thread pitch.  It was wrong, too high, so the hydraulic torqueing up, while correct, was over stretching the studs.
