Early findings put blame on Navy crew for deadly Fitzgerald collision, sources say

Initial findings into the June 17 collision between the Navy destroyer FITZGERALD and a civilian cargo ship in the Philippine Sea reveal that the American crew might have caused the disaster that killed seven U.S. sailors, according to a CNN report Friday. Citing unnamed military sources, CNN said officials believe a string of human errors aboard the guided-missile destroyer’s bridge contributed to the collision with the much larger ACX CRYSTAL, a container vessel. “They did nothing until the last second,” CNN quoted one official as saying. “A slew of things went wrong.” The news organization reported that a second official said the crash “will wind up being our (the U.S. Navy’s) fault.” The interim probe determined that FITZGERALD crew members failed to realize that the ACX CRYSTAL was approaching and that they took no actions needed to avoid the collision, CNN reported. Several multinational investigations continue into the accident, which occurred about 64 miles south of Japan. The container ship speared the Fitzgerald below the destroyer’s waterline, flooding it with seawater and drowning seven sailors. The dead included Yeoman 3rd Class Shingo Alexander Douglass, 25, of Oceanside, and Navy Fire Controlman 2nd Class Carlos Victor Ganzon Sibayan, 23, of Chula Vista. Both were buried with full military honors nearly two weeks ago.Navy officials in San Diego said the Navy’s investigation into the collision continues and that they had no comment on CNN’s report.
