Yep, airline pilots, mariners, the military and emergency services all learn very quickly that things will go wrong, and by trying to anticipate as many of those problems as possible, their reaction time in a crisis is vastly improved - unlike 'automation'.

The only exception I know of in the case of 'automation' is when 3 independent computer systems are used - one on-line, one on active standby (i.e. monitoring the data input), and the 3rd monitoring the other two.  If one goes down, alarms ring and the other two take temporary control until the 'techies can sort out the problem.

Classic example is on ships with the more sophisticated Dynamic Positioning System such as dive ships (where divers live in saturation chambers on board and descend in bells to 400' / 600' from where they work swimming free but using umbilical cables from the bell supplying heat and breathable gasses.  They cannot afford for the ship to wonder off location.

Similarly, deep water exploration rigs depend on dynamic positioning systems to maintain their position over the well.

Pity banks and governments etc don't use multiple computer systems to prevent their frequent computer breakdowns, but that is another story.
