We've got an exclusive investigation into a flagship NHS hospital which opened to great fanfare - but behind the gleaming façade, a host of problems. Two patients on dialysis died after water was found to have 16 times the recommended limit of chlorine. Police found no link - but we've found the contamination was just one of a series of major flaws. Cathy Newman has this exclusive story.
