Mike,

The closest I ever came to a "safety management system" was when Kvaerner Govan in the 90s issued the staff with white hard hats and steel toe capped boots and shoes (still got mine - great in winter). White, and grey, boiler suits (still got both) had always been de rigeur for keeping the office clothes clean when down the ship. Hard hats on new-buildings were mainly for allowing one to bump quickly along alleyways or other paths with lowish doorways.  On one vessel we were issued with small ear plugs that reduced the sound of caulkers from something that reduced grown men to screaming apoplexy, down to a gentle tinkle.  People had had a stiff upbringing, no dilutee crap, and tried very hard not to do stupid things.  At sea, nobody I ever met had to be given a script from which to work. What changed?
