Mike,

I read - "Clinicians will be free to commission services in the way they 
consider best". I am reminded of a TV series (Monty Python?) which had a 
sketch involving an operating theatre with an op in full swing.  "Scalpel!" 
ordered the surgeon, as they do on TV.  One was offered to him.  There 
followed a dialogue about sourcing best value, and someone was despatched to 
fetch a cheaper, but just as good, scalpel, from another supplier.  Thank 
Christ I live up here.
