Probably both, a fatal combination.

Seriously though we, on this site and elsewhere, have often raised doubts
about the stability of these monsters, add a little bit of free surface and
"Whoops, there goes another cruise ship". Add to that untrained crews from
third world countries etc etc. Maybe a wake up call but I wouldn't hold my
breath, they were lucky to get off relatively lightly. A comment on
tonight's news, you probably saw it as well, that these ships are money
making machines and safety comes second.
