a number of newspapers recently quoted John Large, an independent consultant and nuclear engineer, as saying a combination of excess pressure and blocked release valves could have led to the risk of radioactive water leaking, possibly exposing the crew to radiation. 
He said: "This was a serious incident. It illustrates the breakdown of the rigid procedures required to ensure submarines are fit for service. 

"The very fact two submarines were allowed to sea with blank plates fitted... was an accident waiting to happen." 

The Independent said the blocked valves, on the hull of the submarines, meant that steam from nuclear-powered boilers could not have been released in an emergency, leading to a potentially disastrous build-up of pressure.

John Large, a consultant on nuclear safety, said: "It was a very significant failure. These two submarines were unfit for service. It was a perilous situation." 

my question is are these valves located on the pressure hull of the submarine and if so how do they work when the submarine is submerged some hundreds of feet?
