The deep water rigs indeed have BOP's that large. They include a lower package, the preventers, wellhead etc. rest around, the BOP, and the upper riser package. 

I think you may discover the HORIZON toolpusher, BP company man , and driller not on the rig did a no-no and left a drill string fish in the hole, did not cement it in place, tried for three days to recover it, and finally said to heck with it. 

That string was blown up the hole and lodged in the preventers which are designed to seal around one string , not two. Again , human error.
